Eligibility:

By using this website, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions. These terms may be updated or modified from time to time without prior notice, and your continued use of the site constitutes acceptance of any changes.

1. ARMHS services provided by Absolute Health and Wellness LLC are available to individuals who meet the eligibility criteria set forth by the Minnesota Department of Human Services (DHS). This includes being 18 years old or older, having a primary diagnosis of a mental illness, and demonstrating significant impairment in functioning as assessed by a Diagnostic Assessment and a Functional Assessment.

Certification and Recertification:

1.  Absolute Health and Wellness LLC is certified by the Minnesota Department of Human Services (DHS) to provide ARMHS. Certification ensures our capability to deliver the full array of ARMHS services directly or through contracted providers.
2. Absolute Health and Wellness LLC will undergo recertification by DHS every three years to maintain compliance with certification requirements.
3. Additional certification may be required from individual counties where services are provided, as per local health and human services system regulations.

Provider Qualifications:

Absolute Health and Wellness LLC employs qualified professionals eligible to provide ARMHS as outlined by DHS guidelines. These may include Mental Health Professionals, Certified Rehabilitation Specialists, Mental Health Practitioners, Clinical Trainees, Mental Health Rehabilitation Workers, Certified Peer Specialists, Physicians, Registered Nurses, Physician Assistants, and Pharmacists.

Covered Services:

1. ARMHS services provided by Absolute Health and Wellness LLC include but are not limited to Basic Living and Social Skills, Certified Peer Specialist Services, Community Intervention, Functional Assessment, Individual Treatment Plan, Medication Education, and Transition to Community Living Services.
2. All covered services are delivered face to face, except for community intervention, which may include telephone contacts when deemed appropriate.

Non-Covered Services:

1. ARMHS provided by Absolute Health and Wellness LLC does not cover transportation services, household tasks, chores, or activities primarily of a social or recreational nature.
2. Services performed by providers not enrolled with ARMHS, services performed by volunteers, and time spent “on call” without delivering services are not covered.
3. ARMHS does not encompass job-specific skills training, outreach services, or room and board services
   
   

Authorization and Billing:

1. Authorization is required for services exceeding specified limits as detailed in the ARMHS Benefits chart provided by DHS.
2. Absolute Health and Wellness LLC will bill for ARMHS services using the MN–ITS 837P system and adhere to billing guidelines and CPT modifiers as specified by DHS.
3. Request for authorization must be supported by relevant documentation demonstrating medical necessity and adherence to ARMHS guidelines.
   
   

Legal Compliance:

Absolute Health and Wellness LLC operates in compliance with all relevant legal statutes and regulations outlined by the State of Minnesota, including but not limited to Minnesota Statutes 256B.0623, 245I.011.5, and 245I.10.

Modification of Terms:

Absolute Health and Wellness LLC reserves the right to modify these terms and conditions at any time. Changes will be e_ective immediately upon posting on our website. Continued use of our services after such modifications constitutes acceptance of the revised terms.
